
Famed novelist Paul Sheldon (James Caan) has just finished his latest novel. As he departs from Silver Creek,Colorado, he drives in the middle of a blizzard and his car goes off the road. He is rescued by former nurse Annie Wilkes (Kathy Bates) and brought back to her home where she tends to his injuries.
She claims she is his number one fan, and goes on and on about how she cherishes the novels involving the character, Misery Chastaine. At first, Annie seems to be a very kind nurse with a very happy-go-lucky personality but it becomes clear to Paul that she is psychotic. When she is feeding him and reading his new book she disagrees with the profanity; she loses control, spilling some soup on him, but regains control and humbly apologizes for her actions.
She then gets a copy of Paul's already published book entitled, Misery's Child the latest Misery novel and the last, as Paul has decided to go on writing other stories, including the one he has already finished. After he has killed Misery in this story, Annie goes into a rage, almost killing Paul. She claims that she never contacted the hospital or anyone Paul knows. Annie then leaves and Paul decides to escape his room but she has locked the door making it impossible for him to escape. Annie comes to him the following morning and makes him burn his latest novel. She makes him write another Misery novel entitled Misery's Return where he somehow brings the character back to life. As Annie is setting up the equipment, Paul spots a hairpin on the floor. Thinking it will allow him to unlock his door, he sends Annie into town to get him the right kind of paper. While she is away, Paul unlocks his door and tries to call the police but finds out Annie has a fake telephone. He finds a number of Novril pills and takes some, planning to drug her.
As Paul has satisfied Annie with his work on bringing Misery back to life, he asks her if she will have dinner with him; she happily accepts. As they dine, Paul makes Annie get a candle and unsuccessfully drugs her wine. Paul is now clueless as to what to do but continues writing the book.
Meanwhile, local Sheriff Buster (Richard Farnsworth) has been investigating the disappearance of Paul Sheldon. After some careful investigating, he thinks Paul was kidnapped after finding his car and the marks on his door. One night, Annie comes to give Paul his medicine and has a gun with her thinking she might use it. As Annie leaves in the night, Paul decides to get a butcher knife from the kitchen to defend himself. He spots Annie's scrapbook and finds a bunch of articles about her, one in particular was where Annie was head maternity nurse and was on trial for infanticide and was also responsible for other deaths as well but was acquitted. In the middle of the night, Annie drugs Paul and the following morning, ties him up and tells him that she knows he's left his room in her absence and found out he planned to kill her. She then puts a board across his ankles and breaks them with a sledgehammer.
As Buster continues his investigation, he comes across an article with Annie in it and reads a passage quoted by one of the Misery novels, making it clear to him that Annie is involved in Paul's disappearance. As he goes to pay her a visit, Annie drugs Paul and hides him in the basement. As Buster leaves, Paul regains consciousness and tries to get Buster's attention. He finds Paul in the basement but is shot by Annie. Annie decides to end both their lives, but Paul manages to talk Annie out of it by convincing her to let him finish the book. He then takes some lighter fluid planning to set the book on fire.
